Maintenance and Safe Use of Speed Increasing Handpieces
Introduction
Speed increasing handpieces are essential tools in dental practices, designed to enhance precision and efficiency during procedures. Proper maintenance and safe use are crucial for ensuring the longevity of the handpieces and the safety of both the operator and the patient.
Maintenance Instructions
1. Cleaning, Lubrication, and Purging after each use:
o Cleaning: Use a brush and deionised water to clean the external surfaces of the handpiece only. Avoid soaking the handpieces in water or chemicals as this can lead to corrosion.
o Lubrication: Apply lubricant to the drive air tube and the moving parts as recommended by the manufacturer. Oiling the handpiece prior to autoclaving is essential to promote the smooth running of gears and bearings. Failing to do so can significantly impair performance and reduce lifespan as well as cause overheating.
o Purging: The handpiece must be purged of excess oil before autoclaving. Failure to purge excess oil can cause the formation of a slurry during the autoclaving process. This slurry can lead to overheating and premature failure of gears and bearings during use.
2. Sterilization:
o Autoclaving: After cleaning, lubrication, and purging, place the handpiece in an autoclave pouch and sterilize it in an autoclave. Follow the manufacturer's guidelines for temperature and cycle duration.
o Avoid Chemical Sterilants: Do not use chemical sterilants unless specifically recommended by the manufacturer, as these can damage the internal components.
3. Periodic Maintenance:
o Inspection: Regularly inspect the handpiece for signs of wear and tear, such as cracks or loose parts.
We would typically expect our speed increasing bearings to last between 6-12 months dependent on maintenance protocol and usage. Proper maintenance can significantly improve that duration however it is important be aware that speed increasing handpiece have significantly higher maintenance demands and costs than high speed turbines.

o Clinician’s Responsibility: It is the clinician's responsibility to ensure the safe function of each handpiece prior to use on a patient.
o Visual Inspection: Before each use, visually inspect the handpiece for any damage or abnormalities.
o Function Test: Run the handpiece through for 30 seconds to ensure there are no signs of bearing failure such as excessive vibration, noise, or heat. Check that burs rotate properly and about their central axis.
o Initial Heat Dissipation: Be aware that there can be excess heat when the handpiece is first used post-autoclaving as the gears engage and any debris is cleared. Ensuring this excess heat has dissipated prior to use on a patient is essential.
2. Proper Handling:
o Grip and Control: Hold the handpiece with a firm grip to maintain control during use. Avoid applying excessive force, which can damage both the handpiece.
o Avoid Dropping: Handle the handpiece carefully to avoid dropping it, as impacts can misalign the internal components.
3. Operational Precautions:
o Use the Correct Burs: Ensure the burs used are compatible with the handpiece and are properly inserted and secured.
o Speed Settings: Operate the handpiece at the recommended speed settings for different procedures to prevent excessive wear and potential injury.
o Avoid Soft Tissue Contact: Always ensure that the head of the handpiece is not in contact with the patient’s soft tissues, as the handpiece can generate heat and cause burns.
o Bearing Failure Vigilance: Be aware that often, when a speed increasing handpiece bearings fail, the bur will continue to cut, however, excess heat will be generated. Be vigilant for other signs of bearing failure to avoid overheating, which can cause damage to both the operator and the patient.
4. Post-Use Protocol:
o Immediate Cleaning: After use, immediately clean the handpiece to remove debris and prevent build-up.
o Storage: Store the handpiece in a clean, dry place. Use protective caps or cases to prevent contamination and damage.
